Ticketing Order Form  

This form is used to order tickets to be SOLD at the Schine Box Office. You must be a registered student organization, recognized greek chapter, administrative or academic department to request ticket sales at the Schine Box Office. In order to place a ticket sale request, please follow these steps:

1. Click the link below to open the Ticket Order Form (Adobe Acrobat® Required).
2. You can either fill out the form using Acrobat or print it and fill it out manually.
3. Once all form fields are filled out, print the form and obtain all the appropriate signatures.
4. Completed forms should then be hand-delivered to Kim Green, Box Office Supervisor.
